Surgical Treatment of Toxic Megacolon.


Abstract
The term toxic megacolon describes an acute, life-tbreating exacerbation of inflammatory bowel disease distinguished by dilatation of a part or all of the colon. Severe inflammation in the wall of the paralyzed segment of colon is responsible for the patient¢¥s fever, tacbycardia, and other toxic symptoms and signs.
Toxic megacolon is generally considered to be a lethal complication of ulcerative colitis, ; it may also arise as a complication of granulomatous. colitis (Crohn¢¥s colitis), amebic colitis, amebic dysentery, ischemic colitis, pseudomembranous enterocolitis, typhoid fever, salmonella colitis, cholera, ischemic colitis associated with retroperitoneal fibrosis, and pregnancy.
It is characterized radiologically by colonic dilatation, loss of haustral pattern, and mucosal ulceration and pathologically by a transmural necrotizing inflammatory process.
The recommended methods of management range from prolonged conservative treatment to emergency siirgery.
During the period of 1980 to 1981, we have experienced on 4 patients suffering from toxic dilatation of the colon (3 amebic colitis and 1 ulcerative colitis) and have ve operated on 2 patients of amebic colitis and 1 patient of ulcerative colitis, and have had 2 operative deaths in Department of Surgery, Jeonbug National University Hospital.
